OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that they were exposed to fall hazards:  On or about May 27, 2022, the employer did not ensure employees were protected from fall hazards of approximately twelve feet while standing on a metal carpet roll platform carrier that was elevated and supported by a forklift to retrieve products from the top of the warehouse racking system with no means of fall protection systems or devices in place.   Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).

National Carpet Outlet, Inc. was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1333368, citation number 1, item number 3 and was affirmed as a final order on September 25, 2018, with respect to a workplace located at 5730 Cleveland Road Wooster, Ohio 44691.
