29 CFR 1926.454(c)(2): 29 CFR 1926.454(c)(2): The employer failed to train employees where changes in the types of scaffolds, fall protection, falling object protection, or other equipment presented a hazard about which they had not been previously trained:  a) On or about 01/31/2020 at 3700 Narcissus Ave. Northport Industrial Park Sanford, FL, the employer did not train employees on a safe procedure to access and egress from the roof, thereby exposing employees to a 36-foot fall hazard when climbing the guardrails of a scissor lift.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  a. On or about 01/31/2020, at 3700 Narcissus Ave., Northport Industrial Park, Sanford, FL, employees were exposed to a 36-foot fall hazard, in that, fall protection was not used when walking to and from the roof access panel.  b. On or about 01/31/2020, at 3700 Narcissus Ave., Northport Industrial Park, Sanford, FL , employees were exposed to a 36-foot fall hazard, in that, fall protection was not used when climbing through the access panel and stepping onto the guardrails to gain access to and from the scissor lift.
