29 CFR 1910.29(a)(2): The employer did not provide and install all fall protection systems and falling object protection this subpart requires, and comply with the other requirements in this subpart before any employee begins work that necessitates fall or falling object protection:  a)  At the rinsing pit.  On or about February 4, 2020, employees were exposed to fall hazards, while working in the shop without adequate fall protection at the rinsing pit walkway.  b)  Second floor area.  On or about February 4, 2020, employees were exposed to fall hazards, while working in the upper floor of the shop without fall protection.

29 CFR 1910.305(b)(2)(i): All pull boxes, junction boxes, and fittings were not provided with covers identified for the purpose. If metal covers are used, they shall be grounded. In completed installations, each outlet box did not have a cover, faceplate, or fixture canopy:  a) In the lab room, north wall.  On or about February 4, 2020, and at times prior thereto, employees were exposed to electrical hazard while working around the outlets that were missing a faceplate cover.  b) In the lab room, west wall.   On or about February 4, 2020, and at times prior thereto, employees were exposed to electrical hazards while accessing light switches that were missing a faceplate cover.

Stored pressure extinguishers do not require an internal examination. The employer shall record the annual maintenance date and retain this record for one year after the last entry or the life of the shell, whichever is less. The record shall be available to the Assistant Secretary upon request:  Throughout the shop.  On or about February 4, 2020, and at times prior thereto, employees were exposed to fire hazards while working in the shop without the fire extinguishers having been inspected annually.
