29 CFR 1910.22(c): The employer did not provide, and ensure each employee used, a safe means of access and egress to and from walking-working surfaces:   On or about February 5, 2020, and at times prior thereto, the employer did not ensure all ladders and means of access/egress to walking and working surfaces were free from visible hazards and defects and the defects corrected prior to each initial use and each work shift, exposing employees to fall and struck-by hazards where:  a. ladders used for access to machinery, b. ladders used to access the conveyors for maintenance, c. ladders used to access the upper areas of the shredder for maintenance,  d. walkways used to cross the conveyors, and e. walkways used by employees to access work areas throughout the facility.
