29 CFR 1926.404(b)(1)(i): General. The employer shall use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites. These requirements are in addition to any other requirements for equipment grounding conductors. At the 1108 Main Street, Stevens Point, Wisconsin jobsite, a ground fault circuit interrupter nor assured equipment grounding conduct program was utilized when utilizing extension cords to power various equipment.

29 CFR 1926.404(b)(1)(iii)(C): Each cord set, attachment cap, plug and receptacle of cord sets, and any equipment connected by cord and plug, except cord sets and receptacles which are fixed and not exposed to damage, shall be visually inspected before each day's use for external defects, such as deformed or missing pins or insulation damage, and for indications of possible internal damage. Equipment found damaged or defective shall not be used until repaired. At the 1108 Main Street, Stevens Point, Wisconsin jobsite, extension cords used to power construction lifting equipment had missing ground pins. The extension cords were not removed from service exposing employees to electrical shock hazards.

29 CFR 1926.405(a)(2)(ii)(I): Flexible cords and cables shall be protected from damage. Sharp corners and projections shall be avoided. Flexible cords and cables may pass through doorways or other pinch points, if protection is provided to avoid damage. At the 1108 Main Street, Stevens Point, Wisconsin jobsite, extension cords used to lower block heaters went through a block window on the third floor down to an under ground level. The extension cord was not protected from damage from the sharp block opening and the weight of the cord.

29 CFR 1926.850(a): Prior to permitting employees to start demolition operations, an engineering survey shall be made, by a competent person, of the structure to determine the condition of the framing, floors, and walls, and possibility of unplanned collapse of any portion of the structure. Any adjacent structure where employees may be exposed shall also be similarly checked. The employer shall have in writing evidence that such a survey has been performed. On or about December 2019 and thereafter at the 1108 Main Street, Stevens Point, Wisconsin jobsite, a demolition plan was not developed prior to demolition activities of a multistory building.
