29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): At a jobsite located at 1500 Jarboe street, Kansas City, Missouri 64108, an employee was exposed to a serious fall hazard. An employee tasked with installing counterflashing on an exterior wall was not protected from falling by guardrail systems, safety net system, or personal fall arrest system. The employee was exposed to a fall hazard distance of 10 feet 9 inches to the concrete patio below.

29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. Red Hammer Roof Group, LLC. of Kansas City, Missouri did not report an employee hospitalization as a result of a work-related incident to OSHA within twenty-four hours. An incident occurred on or about February 1st 2019 which involved an employee injury caused from a fall from elevation and subsequent hospitalization. The incident was not reported to OSHA until February 3rd, 2019.
