29 CFR 1910.119(e)(6): The employer did not update and revalidate by a team meeting the requirements in paragraph (e)(4) of this section, to assure that the process hazard analysis is consistent with the current process, at least every five (5) years after the completion of the initial process hazard analysis: (a) Crude Unit: On January 27, 2020 and times prior, the employer did not conduct an update and revalidate the facility siting portion of the process hazard analysis at least every five years. The facility siting portion of the process hazard analysis was last revalidated in 2014.
