29 CFR 1910.147(d)(6): Prior to starting work on machines or equipment that had been locked out or tagged out, the authorized employee did not verify that isolation and deenergization of the machine or equipment had been accomplished: (a) Hydrogen Gas Plant Unit (HGP2): On September 6, 2022, employees were attempting to install blinds at the pressure swing adsorption vessel outlets in preparation of changing out a pressure relieve valve. A verification was not conducted to ensure the pressure swing adsorption vessel was isolated resulting in the release of flammable hydrogen gas.
