29 CFR 1910.147(c)(4)(ii): The procedures shall clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, the following:  29 CFR 1910.147(c)(4)(ii)(B): Specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy;  a. On or about February 6, 2020, located in the Bindery Department, the employer did not list the rules and techniques on energy control procedures pertaining to thermal hazards for XtraCoat UV Roller Coater printing press, model number XCF 29-1-3A, serial number CON-1615 that was associated with performing service and maintenance tasks. There were no specific procedural steps for the safe shutdown and isolation of the thermal energy to protect the employees from the UV light heat, thereby exposing employees to burn hazards.

29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence:  a. On or about February 6, 2020, located in the Bindery Department, employees were exposed to machine hazards associated with moving parts, in-going nip points, and thermal hazards during servicing and cleaning the XtraCoat UV Roller Coater printing press, model number XCF 29-1-3A, serial number CON-1615. The employer failed to implement energy control application steps as the machine was not shut down or turned off to perform the servicing [per the 1910.147(d)(2) requirements]. As a result, the remaining applicable energy control elements, involving machine isolation [(d)(3)], LOTO device application [(d)(4)], dissipation of residual energy [(d)(5)(i)], and verification of isolation [(d)(6)], were not implemented to protect employees from machine servicing hazards.  On February 6, 2020, an employee bypassed a guard to the lockout activity of cleaning print rollers on the  XtraCoat UV Roller Coater printing press, model number XCF 29-1-3A, serial number CON-1615. Lockout/tagout was not performed prior to cleaning the rollers resulting in a fingertip injury.

29 CFR 1910.147(c)(6)(i): The employer shall conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard are being followed.   a. On or about February 6, 2020, located in the Bindery Department, the employer did not conduct a periodic inspection of the energy control procedures at least annually, thereby exposing employees to the hazards associated with servicing and maintenance activities on machines and equipment, i.e. electrical shock/electrocution, caught-in/caught-between hazards, etc.

29 CFR 1910.147(c)(7)(i)(A): Each authorized employee shall receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control.   a. On or about February 6, 2020, located in the Bindery Department, employees performed service and maintenance activities on machines and equipment without receiving authorized employee lockout/tagout training. There employees were not able to recognize hazardous energy sources, type and magnitude of the energy available, and the methods and means for energy isolation and control exposing the employees to the unexpected energization or startup of the equipment or release of hazardous energy.
