29 CFR 1910.119(e)(1): The process hazard analysis was not appropriate to the complexity of the process and did not identify, evaluate, and control the hazards involved in the process:   On or about February 7, 2020 and at times prior thereto at the Big R Gas Plant in Powell, Texas, when the employers PHA did not to identify, evaluate, and control the hazards associated with the flare and knockout system. This condition exposed workers to hazards including but not limited to fire, explosion, and inhalation of hazardous materials.

On or about February 7, 2020, and times prior thereto, at the Big R Gas Plant in Powell, Texas, the employer did not implement required action items from the 2010 process hazard analysis to evaluate and control the associated hazards of the process, exposing workers to fire, explosion and inhalation hazards.   (a) The employer did not implement the action item from the 2010 hazard assessment to perform PHA of the HP and LP flare systems. (b)  The employer did not implement the action item from the 2010 hazard assessment to perform PHA of the Inlet Compression System. O'Ryan Oil & Gas was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.119(j)(2), which was contained in OSHA inspection number 1357199, citation number 01, item number 7/a and was affirmed as a final order on April 22, 2019, with respect to a workplace located at 8601 NE CR 2100, Powell, Texas 75153.
