29 CFR 1926.451(a)(6): Scaffolds were not designed by a qualified person:  On or about February 11, 2020 a wood framed scaffold was not designed or erected by a qualified person, exposing the employee to the hazards associated with scaffold use, such as a collapse or falls.    Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:   On or about February 11, 2020 an employee performing residential construction activities was not protected from a fall while working from a wood framed scaffold, exposing the employee to a fall of 21 feet.   Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b).  On or about February 11, 2020 an employees performing residential construction activities were not protected from a fall, exposing the employees to a fall of 23 feet.  Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.   On or about February 11, 2020 employees used a portable "A" frame ladder to access the second floor.  The ladder was two feet short of the upper landing, and did not extend three feet past the landing, exposing employees to 8 foot fall hazards.   Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).
