29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use:    (a)Custom Fabricators, Inc. at 1900 14th Avenue Northwest, Watertown, SD 57201: On or about and at times prior to February 11, 2020, the employer did not ensure that employees were protected from chemical hazards and hazards associated with respirator use in that, the company had not established and implemented a written respiratory protection program covering employee use of respirators. This condition exposed employees to chemical hazards and hazards associated with respirator use.    Abatement Note:  The following provisions of this section include:  (i) Procedures for selecting respirators in the workplace;  (ii) Medical evaluations of employees required to use respirators;  (iii) Fit-testing procedures for tight-fitting respirators;   (iv) Procedures for proper use of respirators in routine and reasonably foreseeable emergency situations;  (v) Procedures and schedules for cleaning, disinfecting, storing, inspecting, repairing, discarding, and otherwise maintaining respirators;  (vi) Procedures to ensure adequate air quality, quantity, and flow of breathing air for atmosphere-supplying respirators;  (vii) Training of employees in the respiratory hazards to which they are potentially exposed during routine and emergency situations.  (viii) Training of employees in the proper use of respirators, including putting on and removing them, any limitations on their use, and the maintenance; and  (ix) Procedures for regularly evaluating the effectiveness of the program

The employer did not develop practices and train employees to prevent electric shock or other injuries when work is performed near or on equipment or circuits which are or may be energized. This condition exposed employees to hazards such as burns, shock, electrocution, arc blast, and arc flash.      Abatement Note: Training requirements are found in 29 CFR 1910.331 through 29 CFR 1010.335. The training required by this section shall be of the classroom or on-the-job type. The degree of training provided shall be determined by the risk to the employee.

An employee installing a 70-amp breaker in Panel #5, turned off the panel's main breaker but did not de-energize the panel prior to installing the breaker. The employee was exposed to live electrical components associated with working within Panel #5 when removing the dead front cover to install the breaker. This condition exposed employees to burns, shock, and electrocution hazards.
