29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a)Custom Fabricators, Inc. at 1900 14th Avenue Northwest, Watertown, SD 57201: On or about and at times prior to February 21, 2023, the employer did not ensure that the Cincinnati Metal Shear Model: 1810, Serial number: 11035, front guard was properly adjusted to protect from operator contact with the shear blade and where warning barrier chains were not installed on the back of the machine while the machine was in use. This condition exposed operators to struck by, crushing, and amputation hazards. (b)Custom Fabricators, Inc. at 1900 14th Avenue Northwest, Watertown, SD 57201: On or about and at times prior to February 21, 2023, the employer did not ensure that the Top Tech Manual Lathe Model: 280x700, was equipped with a properly installed guard over the lathe chuck of the machine. This condition exposed operators to caught-in and amputation hazards.

This condition exposed operators to caught-in and amputation hazards.

This hazard exposed employees to crushing and amputation hazards. (b)Custom Fabricators, Inc. at 1900 14th Avenue Northwest, Watertown, SD 57201: On or about and at times prior to February 21, 2023, the employer did not ensure the point of operation on the 100-Ton Accurpress CNC hydraulic press brake (serial# 2789) was properly guarded to prevent employees from having any part of the their body in the danger zone during machine operations. This hazard exposed employees to crushing and amputation hazards.
