29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    (a) Turbo Press, Inc. at 100 Lipan St. Denver, CO 80223: On or before 2/11/2020 the employer did not establish an energy control program in accordance with this standard. This condition exposed employees to amputation and crushing hazards.

A rotating counter die drum was exposed during the die cutting cycle.

29 CFR 1910.303(g)(1)(i): The dimension of the working space in the direction of access to live parts operating at 600 volts or less and likely to require examination, adjustment, servicing, or maintenance while alive was less than indicated in Table S1:    (a) Turbo Press, Inc. at 100 Lipan St. Denver, CO 80223: On or before 2/11/2020 the employer did not ensure that items were stored at least 30 inches away from the faces of the electrical panels in the printshop. A work station including a desks, a chair and other items including paper were stored in front of several 240v panels, which were also not closed. This condition exposed employees to electric shock and fire hazards.
