29 CFR 1910.134(m)(1): Records of medical evaluations required by 29 CFR 1910.134 were not retained and made available in accordance with 1910.1020:(a) (LOCATION) (IDENTIFY SPECIFIC OPERATION/CONDITION) (DESCRIBE HAZARD)  Decor Cabinets Southeast, LLC, Gainesville, GA - An employee was required to use a disposable dust respirator N95 while cutting wood, using glue and during assembly. The written recommendation from the physician was not made available by the employer after the medical evaluation was conducted, on or about 2/13/20.  Decor Cabinets Southeast, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.134(m)(1), which was contained in OSHA inspection number 1377005, citation number 1, item number 2d and was affirmed as a final order on 12/29/20, with respect to a workplace located at 1975 Delta Drive, Gainesville, GA.
