29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    15 Coppage Dr, Worcester, MA: On February 19, 2020 and at times prior, the employer had not established an energy control program to protect employees who set up and maintained single energy source braider machines and dual source buncher machines.  There were no written energy control procedures for the Buncher or other multi-energy source machines.  Maintenance employees received only on the job training to isolate machines when working on them and authorized, affected and other employees had not received training on their responsibilities relating to hazardous energy control.  The employer had not performed any annual periodic inspections that ensured each procedure was inspected for inaccuracies during an actual energy control task.  Additionally, the employer did not have the inspector review with each authorized employee their responsibilities under each energy control procedure they were authorized to use.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    15 Coppage Dr, Worcester, MA: On February 4, 2020 and at times prior, employees operated the incompletely guarded Buncher 6 cable machine and employees were exposed to caught-in hazards where cables and filler materials are drawn through unguarded dies.  On February 4, 2020, an employee received a finger injury when it was pulled into the unguarded die area of the Buncher 6 machine.
