29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes by covers:  (a) Belair Sitework Services at 1600 12th St. Denver, CO 80204: On or before 2/18/20 the exposing employer did not protect employees from injury in that employees were fine grading around nine unguarded manholes. The steel manhole covers were nearby on the ground and were not replaced prior to commencing work. Nine manholes in the area were covered with thin plastic sheeting. No other methods of affixing a protective cover were performed. An employee fell through a manhole into a cistern filled with water. This condition exposed employees to fall and drowning hazards.

29 CFR 1926.503(a)(2)(i): Employees were not trained, as necessary, by a competent person qualified to recognize the nature of fall hazards in the work area:   (a) Belair Sitework Services at 1600 12th St. Denver, CO 80204: On or before 2/18/20 the exposing employer did not protect employees from injury in that employees worked in the vicinity of nine unguarded manholes without being trained on the fall hazards associated with their work area. The steel manhole covers were nearby on the ground and were not replaced prior to commencing work. The manholes in the area were covered with thin plastic sheeting. No other methods of affixing a protective cover were performed. A laborer working near the manholes fell through an unguarded manhole opening and drowned. This condition exposed employees to a fall hazard greater than six feet in addition to a drowning hazard.
