29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    (a) Production area: On or about February 12, 2020 and times prior thereto, employees were exposed to ingoing nip points from pinch rollers, including but not limited to the Lown Model B-400.     (b) Production area: On or about February 12, 2020 and times prior thereto, employees were exposed to ingoing nip points from pinch rollers, including but not limited to the Hendley & Whittemore pinch roller S/N 14963.    (c) Production area: On or about February 12, 2020 and times prior thereto, employees were exposed to ingoing nip points from pinch rollers, including but not limited to the Peck Stow & Wilcox Co. Model 381-D roll former.     (d) Welding area: On or about February 12, 2020 and times prior thereto, employees were exposed to amputation hazards when operating a 45 ton hydraulic press to press carbon steel cones and piping.    (e) Welding area: On or about February 12, 2020 and times prior thereto, employees were exposed to amputation hazards when operating a 50 ton hydraulic press to press carbon steel cones and piping.
