29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: (a) On March 10, 2021 and times prior thereto, employees were exposed to forklift hazards while operating the Caterpillar forklift, S/N 12X2232 with an attachment used to move material such as but not limited to 3/8" sheet metal. The attachment has not been approved by the manufacturer or evaluated to see how it affects the capacity and safe operation of the forklift. (b) On March 10, 2021 and times prior thereto, the employer did not have written approval to perform spot welds on the carriage for the Caterpillar forklift, S/N 12X2232 affecting the safe operation of the equipment.
