29 CFR 1926.102(a)(2): The employer did not ensure that each affected employee used eye protection that provided side protection when there is a hazard from flying objects.    On or about February 19, 2020, an employee was not wearing eye protection when using a handheld grinder to cut roofing tile.  Employee was exposed to the hazard of having particles getting into his eye.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b):     On or about February 19, 2020, employees were working on a roof of a home under construction.  The employees were not using any type of fall protection.  Employees were exposed to a fall hazard greater than 6 feet.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    On or about February 19, 2020, the ladder employees were using to access the roof did not extend at least 3 feet above the edge of the roof line.  Employees were exposed to the hazard of falling.

29 CFR 1926.1153(d)(2)(i): The employer did not assess the exposure of each employee who was or may reasonably be expected to be exposed to respirable crystalline silica at or above the action level in accordance with either the performance option in paragraph (d)(2)(ii) or the scheduled monitoring option in paragraph (d)(2)(iii) of this section:    On or about February 19, 2020, at the construction site; an employee was working on the roof of a home under construction cutting roofing tiles using a hand held grinder.  The employee was in a cloud of silica dust from cutting the roofing tile,  the  employer did not assess the exposure of each employee who was or may reasonably be expected to be exposed to respirable crystalline silica.  The employee was exposed to respiratory health hazards.
