29 CFR 1926.102(a)(2): The employer did not ensure that each affected employee used eye protection that provided side protection when there is a hazard from flying objects.  At the residential worksite located at 2920 NW Thoreau Drive, Lee's Summit, Missouri:  Employees were exposed to struck by hazards when using saws and pneumatic nail guns without the use of eye protection.

29 CFR 1926.404(b)(1)(iii)(C): The employer did not ensure that each cord set, attachment cap, plug and receptacle of cord sets, and any equipment connected by cord and plug, except cord sets and receptacles which are fixed and not exposed to damage, were visually inspected before each day's use for external defects, such as deformed or missing pins or insulation damage, and for indications of possible internal damage.  At the residential worksite located at 2920 NW Thoreau Drive, Lee's Summit, Missouri: Employees were exposed to electrical shock hazards in that an extension cord used to power tools onsite was missing a ground pin.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws:  At the residential worksite located at 2920 NW Thoreau Drive, Lee's Summit, Missouri:  Employees were exposed to electrical shock hazards in that the air compressor used to power tools onsite had exposed wiring.  The strain relief had been pulled.

29 CFR 1926.501(a)(2): The employer did not determine if the working/walking surfaces on which its employees were to work had the strength and structural integrity to support the employees safely:   At the residential worksite located at 2920 NW Thoreau Drive, Lee's Summit, Missouri:  Employees were exposed to fall hazards while setting up siding on the exterior of the structure  Employee were standing on plywood laid across the wooden beams on the deck.  Additional employees were using a 6 foot step ladder on the plywood decking.  The plywood decking had not been fastened to the structure.

At the residential worksite located at 2920 NW Thoreau Drive, Lee's Summit, Missouri:  Employees were exposed to fall hazards of greater than 8 feet when performing framing activities without the use of fall protection.
