29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  Employees were exposed to struck by hazards. Employee(s) performing roofing activities with the use of a pneumatic nail guns were exposed to eye injury in that employee(s) were not using eye protection.  Hernandez Construction & More LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number 1256133 citation number 1, item number 1 and was affirmed as a final order on 2/5/2018, with respect to a workplace located at 5325 W 6th St., Lawrence, KS 66049.

Employees engaged in roofing activities were exposed to fall hazards of approximately 10 to 25 feet to the ground. Employees were not provided with fall protection measures.  Hernandez Construction & More LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13) which was contained in OSHA inspection number 1256063, citation number 1, item number 1 and was affirmed as a final order on 2/5/2018, with respect to a workplace located at 5325 W 6th St., Lawrence, KS 66049.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed  Employees were exposed to fall hazards. Employees performing roofing work were exposed to fall hazards of approximately 25 feet to the ground. The ladder used to access the roof did not extend at least 3 feet above the landing surface.   Hernandez Construction & More LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.1053(b)(1) which was contained in OSHA inspection number 1277560 citation number 2 item number 1 and was affirmed as a final order on 5/22/2018, with respect to a workplace located at 5900 Queens Rd., Lawrence, Kansas 66049.
