Test voltages and/or the maximum intervals between tests were not conducted in accordance with Table I-4 and Table I-5. (a) On or about 02/20/2020 - 176 Cochran Road SW, Huntsville, AL, employees were exposed to arc-flash and electrical shock hazards when inspecting and measuring live parts up to 480V and the rubber insulating gloves had not been subjected to a periodic electrical test every 6 months after first issue.
