29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level: (a) On or about February 20, 2020, employees who were in the process of erecting concrete forms for an elevator shaft, were working approximately 10 feet above their fall protection anchor points, resulting in an employee free falling approximately 10 feet when his positioning device failed.

29 CFR 1926.502(e)(9): Positioning device systems were not inspected prior to each use for wear, damage, and other deterioration, and defective components were not removed from service: (a) On or about February 20, 2020, employees who were in the process of erecting concrete forms for an elevator shaft in excess of 20 feet above the lower working surface, were utilizing positioning device systems that were not inspected before use, resulting in an employee falling to the lower working surface because of defective equipment that failed during use.
