29 CFR 1910.134(k)(6): The employer did not provide the basic advisory information on respirators, as presented in Appendix D of 29 CFR 1910.134, in written or oral format to employees who wear respirators when such use was not required by the employer: In the Spiral Building, at the South Side Sampling area, employees are provided voluntary use of filtering facepiece (dust mask) respirators for use while operating machinery and equipment: a) Employees who stick weld Shielded Metal Arc Welding (SMAW) and Gas Metal Arc (MIG) weld inside of 24 to 42-inch steel pipes. This condition exposed employees to gas inhalation hazards. b) Employees who operate the CNC Plasma Cutter that automatically cuts including but not limited to 42-inch steel pipes. This practice exposed employees to inhalation hazards.
