29 CFR 1926.304(d): When the tool was withdrawn from the work, the lower guard did not automatically and instantly return to the covering position. a) On or about February 25, 2020; on the east side of the worksite, employees placed wooden wedges in the guards of a Bosch circular saw to hold the guards back while cutting plywood paneling on the roof.

29 CFR 1926.404(b)(1)(iii)(C): Where an assured equipment grounding conductor program was being utilized, the employer did not ensure that each cord set, attachment cap, plug and receptacle of cord sets, and any equipment connected by cord and plug, except cord sets and receptacles which are fixed and not exposed to damage, were not visually inspected before each day's use for external defects, such as deformed or missing pins or insulation damage, and for indications of possible internal damage. a) On or about February 25, 2020; on the east side of the worksite, employees were using an extension cord with a melted outer shield to power a Bosch circular saw exposing employees to electrical shock.
