29 CFR  1926.501(b)(2)(ii): Each employee on a walking/working surface six feet (1.8 m) or more above a lower level where leading edges were under construction, but who was not engaged in the leading edge work, was not protected from falling by a guardrail(  a) Worksite, 319 Santos Street, East McKeesport PA 15035, On or about March 27, 2023 - Employees were engaged in total roof replacement while near the edge of the roof exposed to approximate 24 feet fall hazards from eave to ground without  the use of a full fall arrest system.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) Worksite, 319 Santos Street, East McKeesport PA 15035, On or about March 27, 2023 - Employees accessed  the second level roof with a portable ladder that was not extended 3 feet (36 inches) above the upper surface while performing a total roof replacement.
