Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. Location: 300 Madison Ave Morristown, NJ 07960 - 2nd Floor Parking Garage a) Employees exposed to fall hazards while conducting demolition work on a concrete stairway platform approximately 10 feet 2 inches above lower level without fall protection resulting in an injured employee. Incident occurred on or about 2/24/2020.

29 CFR 1926.850(a): Prior to permitting employees to start demolition operations, an engineering survey was not made, by a competent person, of the structure to determine the condition of the framing, floors, and walls, and possibility of unplanned collapse of any portion of the structure. Any adjacent structure where employees may be exposed shall also be similarly checked. The employer did not have in writing evidence that such a survey has been performed. Location: 300 Madison Ave Morristown, NJ 07960 - 2nd Floor Parking Garage a) Employer did not provide an engineering survey for the worksite to address hazards resulting in an injured employee. Incident occurred on or about 2/24/2020.
