29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c): On or about 3/2/2020, during catch basin installation, located at the corner of Hilliard Blvd and Wildflower Drive in Rocky River Ohio, the employer failed to ensure that employees were protected from cave-in hazards. The trench was about five feet and two inches (5' 2") deep and the west side of the trench was not benched and/or sloped. Catts Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.652(a)(1) which was contained in OSHA inspection number 1305799, citation number 1, item number 2, and was affirmed as a final order on 4/27/2018, with respect to a workplace located at E. Wallings Road and Wright Street in Broadview Heights, Ohio. Catts Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.652(a)(1) which was contained in OSHA inspection number 1405983, citation number 2, item number 2, and was affirmed as a final order on 7/17/2019, with respect to a workplace located at 929 Pearl Road in Brunswick, Ohio.
