29 CFR 1910.28(b)(1)(i): Each employee on a walking/working surface having an unprotected side or edge which was four feet (1.2m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: On or about 03/02/2020, Production Castings Inc., located at 1410 W. Lark Industrial Drive, Fenton, MO, had floor hole opening(s) which were not adequately guarded in their Remelt area(s). Regardless of height, if a worker can fall into or onto dangerous machine or equipment (such as a conveyor), employers must provide guardrails and toe boards to prevent workers from falling and getting injured.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about 03/02/2020, Production Castings Inc., located at 1410 W. Lark Industrial Drive, Fenton, MO, were exposed to hazards such as those created by the rotating table of the Predator palletizer (S/N: H518-059-31496, Asset No.: 02687). This was located in the shipping and wrapping area of building 1410. During the unmanned process with this palletizer, there were forklifts and pedestrian traffic at or near the machine.
