29 CFR 1910.29(b)(1): (1) The top edge height of top rails, or equivalent guardrail system members, were not 42 inches (107 cm), plus or minus 3 inches (8 cm), above the walking-working surface. On or about 01/26/2022, Production Castings Inc. had not ensured top rail was 39-45 inches above the walking working surface in the aluminum trim press department. The employer installed a guardrail over an opening for the reclaim conveyor. The guardrail was approximately 25 inches high, which exposed employees to a fall hazard.

On or about 1/26/2022, Production Castings Inc. had not ensured a defective powered industrial truck was removed from service until restored to a safe operating condition. An employee was operating a EKKO Forklift - Model: EK15A, S/N: Ek2101B0065. The forklift's brake, reverse and warning lights were missing or broken/inoperable.
