29 CFR 1910.22(d)(1): (1) Walking-working surfaces are inspected, regularly and as necessary, and maintained in a safe condition: On March 3, 2020, located on the south side of a residential structure, employees exposed to a fall hazard while standing on a low pitch roof repairing a solar panel as there were no regular and/or necessary inspections being performed to ensure the work-site, materials, and equipment such as a fall protection systems and/or devices were being maintained and utilized in a safe condition. Potential fall height measured to be greater than 25' with a roof pitch that measured to be 4 in 12.

29 CFR 1910.28(b)(15): (15) Walking-working surfaces not otherwise addressed. Except as provided elsewhere in this section or by other subparts of 29 CFR part 1910, the employer must ensure each employee on a walking-working surface 4 feet (1.2 m) or more above a lower level is protected from falling: A) Guardrail systems; B) Safety net systems; or C) Personal fall protection system, such as personal fall arrest, travel restraint, or position systems. On March 3, 2020, located on the south side of a residential structure, employees were exposed to a fall hazard while standing on a low pitch roof repairing a solar panel while not utilizing a fall protection system. Potential fall height measured to be greater than 25' with a roof pitch that measured to be 4 in 12.
