29 CFR 1910.30(a)(1):     (a) Fall hazards. (1) Before any employee is exposed to a fall hazard, the employer did not provide training for each employee who uses personal fall protection systems or who is required to be trained as specified elsewhere in this subpart.  Employers must ensure employees are trained in the requirements of this paragraph on or before May 17, 2017.    a) On or about March 4, 2020, at the above located workplace, employees in the final assemble/car body and truck assembly marrying area were required to work on an open-sided floor (having pits) which did not have a guardrail system, safety net system, or personal fall arrest system. The employees were thereby exposed to a fall hazard of approximately 5.3 feet.        b) On or about March 4, 2020, at the above located workplace, employees in the commissioning shop were required to work on an open-sided floor (having pits) which did not have a guardrail system, safety net system, or personal fall arrest system. The employees were thereby exposed to a fall hazard of approximately 5.3 feet.    Employees were not trained in the hazards of working around pits.

29 CFR 1910.335(a)(1): Employees working in areas where there were potential electrical hazards were not provided with, and/or did not use, electrical protective equipment that was appropriate for the specific parts of the body to be protected and for the work to be performed:     a) On or about February 21, 2020, at the above located workplace, employees in the assembly area were required to work with an energized rail car and rail car equipment. At least one high amperage battery was not isolated. An arc flash occurred and knocked an employee to the floor. The employees were not provided and required to use electrical personal protective equipment such as eye and face protection, tested rubber gloves with protective outer leather gloves and arc flash resistant clothing that was appropriate for the specific parts of the body to be protected and for the work to be performed. Employees were exposed to the hazards associated with electrical arc flash.    b) On or about late January or early February 2020, at the above located workplace, employees in the assembly area were required install a large, high amperage battery. As they installed the battery, there was an arc flash. The employees performing this work were not provided and required to use electrical personal protective equipment such as eye and face protection, tested rubber gloves with protective outer leather gloves and arc flash resistant clothing that was appropriate for the specific parts of the body to be protected and for the work to be performed. Employees were exposed to the hazards associated with electrical arc flash.

29 CFR 1904.4(a): The employer did not record each work-related fatality, injury or illness case that resulted in the general recording criteria on the OSHA Form 300 or equivalent.       a) On or about November 12, 2019, a recordable injury occurred when an employee fell in the parking lot. No information related to the injury was not entered on the 2019 calendar year OSHA 300 injury and illness log.
