29 CFR 1910.28(b)(1)(i): Except as provided elsewhere in this section, the employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the following: Guardrail systems, safety net systems, or personal fall arrest systems. a. Pit #2 in the warehouse - On or about December 27, 2023, an employee was using a crossing wooden bridge to cross the open pit, when the employee fell approximately fifty-six (56) inches into the pit. The employer failed to ensure that employees were protected from fall hazards by a guardrail system, safety net system or person fall arrest system.
