Combined Systems, Inc., 388 Kinsman Rd, Jamestown, PA 16134, Building D-2, on or about February 25, 2020 and times prior thereto: Employee operates an Enco Variable Speed Mini Lathe to perform work operations including, but not limited to, cutting down brass tubes. The lathe was not equipped with a chuck guard.

Combined Systems, Inc., 388 Kinsman Rd, Jamestown, PA 16134, Building D-2, on or about February 25, 2020 and times prior thereto: Employees operate a dual palm button Atlas air press (pancake press) for work operations that include, but are not limited to, consolidating powder into brass delay tubes. The pancake press is actuated by a two-hand trip device and potentially exposes employee to an amputation/crush hazards due to the delay and slow speed of the machine cycle.

29 CFR 1910.305(g)(1)(iv): Flexible cords and/ or cables were used for purposes prohibited by paragraphs (A) through (F) of 29 CFR 1910.305(g)(1)(iv): A.) Combined Systems, Inc., 388 Kinsman Rd, Jamestown, PA 16134, Building D-2, on or about February 25, 2020 and times prior thereto: A flexible cord was plugged into an electrical receptacle, running down the wall, and across the floor to a second flexible cord which was powering an electrical knockout box and subsequently powered equipment including, but not limited to, a Uline scale on the table. The use of the flexible cords were prohibited in that they were used in lieu of permanent wiring of the structure. B.) Combined Systems, Inc., 388 Kinsman Rd, Jamestown, PA 16134, Building D-2, on or about February 25, 2020 and times prior thereto: A flexible cord was ran to a metal knockout box that was on the floor of the building. The metal knockout box powered equipment including, but not limited to, a Fisher Scientific IsoTemp Incubator (M/N: 550D, S/N: 60500062). The use of the flexible cords were prohibited in that they were used in lieu of permanent wiring of the structure.

The metal knockout box powered equipment including, but not limited to, a Fisher Scientific IsoTemp Incubator (M/N: 550D, S/N: 60500062). The metal knockout box is not designed for portable applications as a cord cap at the end of a flexible cord, and is required to be mounted.
