29 CFR 1910.22(d)(2): The employer did not ensure hazardous conditions on walking-working surfaces were corrected or repaired before an employee used the walking-working surface again.  When the correction or repair cannot be made immediately, the hazard was not guarded to prevent employees from using the walking-working surface until the hazard is corrected or repaired.   On or about March 3, 2020 and at times prior thereto, the employer did not ensure the hazards such as, the damaged ceiling, loose boards, and protruding nails on the walking-working surfaces in the lumber storage sheds were corrected or repaired prior to employees accessing the walking-working surface.  This exposed the employees to a fall hazard:  Instance 1 Shed side # 7   Instance 2 Shed side # 8

The employer did not ensure each flight of stairs having at least 3 treads and at least 4 risers was equipped with stair rail systems and handrails per Table D-2; stair width less than 44 inches (1.1 m), and two open sides required to have one stair rail system on each open side.      On or about March 3, 2020 and at times prior thereto, the employer did not ensure the stairs employees use to access the second story of the lumber storage sheds had a safe stair rail system.  This exposed the employees to fall hazard.

On or about March 3, 2020 and at times prior thereto, the employer did not ensure that the powered industrial trucks (PIT) used by employees were inspected prior to use and taken out of service if defects were identified.  This exposed employees to fall, struck-by, and crush-by hazards:    Instance 1)  Hyster Forklift with hydraulic leak and inoperable alarm and warning lights.    Instance 2)  Toyota Forklift with non-working seat belt and inoperable alarm and warning lights.

On or about March 3, 2020 and at times prior thereto, the employer did not ensure that the powered industrial trucks (PIT) equipment operators were trained and evaluated to recognize the hazards that are associated with operation of PITs. This exposed employees struck-by and crushed-by hazards.
