Canton, Ohio 44720, the employer did not conduct frequent and regular inspections of the job site where employees were exposed to fall hazards (no fall protection while working on the roof) (portable ladders did not extend 3 feet above the upper landing surface) and struck by hazards (employees working installing material with air driven nail guns) from working surfaces of heights approximately 10.1 feet.

This most recently occurred on or about March 5, 2020 at the job site located at 7947 Chatham Ave NW, N. Canton, Ohio 44720; during the installation of roofing material on a residential roof, the employer did not ensure portable ladders were extended 3 feet above the upper landing surface.
