On or about April 22, 2020, an employee was exposed to a 52.5 inch fall hazard above dangerous equipment while standing on the upper platform of a Zero Cell braiding machine with no guardrail system in place.

29 CFR 1910.28(b)(11)(ii): Each flight of stairs having at least 3 treads and at least 4 risers is equipped with stair rail systems and handrails as follows: For a stair width less than 44 inches (1.1 m) and two open sides, one stair rail system on each open side is required. a.) On or about April 22, 2020, and employee was exposed to a 52.5 inch fall hazard while using a flight of stairs located on the second level of a Zero Cell braiding machine that does not have a stair rail on the both open sides.

29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc. (a) On or about 4-22-2020, an employee was exposed to an ingoing nip-point created by overhead revolving drums while operating an inadequately guarded Zero Cell braiding machine.
