29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) On or about March 3, 2020 the employer failed to ensure that effective machine guarding was in place on the stacking area of line F5. Employees are exposed to the pinch points created by the movement of the machine components of the stacker. b) On or about March 3, 2020 the employer failed to ensure that effective machine guarding was in place on the stacking area of line F6. Employees are exposed to the pinch points created by the movement of the machine components of the stacker. c) On or about March 3, 2020 the employer failed to ensure that effective machine guarding was in place on the stacking area of line F9. Employees are exposed to the pinch points created by the movement of the machine components of the stacker.

29 CFR 1910.147(c)(4)(ii)(B): The energy control procedure did not clearly and specifically specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy: On or about March 3, 2020 the employer failed to ensure that employees were protected from the unexpected release of hazardous energies while performing service and maintenance of production line F6. Employees performing monthly maintenance were not protected from all hazardous energies present on the equipment including the pneumatic energy.
