29 CFR 1910.147(c)(7)(i): The employer did not provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees:   East Plant Bldg., 813 E. Locust Street, DeKalb, IL, DB8 Wire Draw Machine.  On or about March 5, 2020, an employee who performed servicing and maintenance tasks, such as setup, was exposed to amputation and crushing hazards to hands and fingers when pulling copper wire through the dies and wheels (capstan).  The employee was not instructed to deenergize and lockout the machine prior to performing setup.  The employee used a foot-controlled "jog" pedal to rotate the wheels forward as the wire was wound.  He then used both hands to remove and reposition the wire on the wheel, with his foot next to the foot pedal.  The employee's hands were exposed to getting caught between the wire and wheel in the event of inadvertent activation of the foot pedal, unexpected startup or release of stored energy.  The employer did not provide training to ensure that the knowledge and skills required for the safe application, usage, and removal of the energy controls were acquired by employees.  Alternate protective measures, such as machine guarding, were not adequate.   Abatement documentation is required for this item in accordance with the requirements of 1903.19(d).

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):     East Plant Bldg., 813 E. Locust Street, DeKalb, IL, DB8 Wire Draw Machine.  On or about March 5, 2020, an employee who performed servicing and maintenance tasks, such as setup, was exposed to amputation and crushing hazards to hands and fingers when pulling copper wire through the dies and wheels (capstan).  The employee was not required to deenergize and lockout the machine during setup.  The employee used a foot-controlled "jog" pedal to rotate the wheels forward as the wire was wound.  He then used both hands to remove and reposition the wire on the wheel, with his foot next to the foot pedal.  The employee's hands were exposed to getting caught between the wire and wheel in the event of inadvertent activation of the foot pedal, unexpected startup or release of stored energy.  Effective alternate protective measures, such as machine guarding, were not adequate.   Employees were not required to control hazardous energy sources in the following sequence:         1) turn off and shut down the machines,        2) physically locate and operate energy isolating devices in such a manner as to isolate the machines or equipment from the energy source(s),  3) affix lockout or tagout devices to each energy isolating device,        4) relieve, disconnect, restrain and otherwise render safe all hazardous residual or stored energy,      5) verify that isolation and deenergization of the machines or equipment have been accomplished.         Abatement documentation is required for this item in accordance with the requirements of 1903.19(d).
