29 CFR 1910.1200(e)(1): The employer did not develop, implement, and maintain at each workplace, a written hazard communication program which described how the criteria specified in 29 CFR 1910.1200 paragraphs (f), (g), and (h) for labels and other forms of warning, safety data sheets, and employee information and training would be met.  This requirement is applicable to construction work per 29 CFR 1926.59:  a) Jobsite: 965 Frankford Ave, Philadelphia, PA 19125 - The employer did not have a written hazard communication program at the workplace.  Employees used hazardous chemicals such as, but not limited to, Sodium Hydroxide, contained in Sureclean.   Noted on or about 3/6/20

This requirement is applicable to construction work per 29 CFR 1926.59:  a) Jobsite: 965 Frankford Ave, Philadelphia, PA 19125 - The employer did not maintain at the jobsite copies of the required safety data sheets for each hazardous chemical.  Employees used hazardous chemicals such as, but not limited to hexavalent chromium, quartz (crystalline silica), etc. contained in Lehigh Heidelberg Cement Group Type N Masonry Cement. 29 CFR 1926.451(g)(1)(vii): For all scaffolds not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, each employee was not protected by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section:  a) Jobsite: 965 Frankford Ave, Philadelphia, PA 19125 - Employees conducting masonry operations from supported tubular welded frame scaffolds were exposed to fall hazards, approximately 40 feet above ground level, without the use of a fall protection system.  Noted on or about 3/6/20.  The employer was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1147076, citation number 1, item number 1, and was affirmed as a final order on 7/14/17, with respect to a workplace located at 450 Fairmount Ave, Philadelphia, PA 19123.
