Employees were engaged in stucco activities and were working on different levels of the scaffold; thus exposing employees to struck-by hazards. Condition was observed on or about 15 SEP 2020.

29 CFR 1926.454(a): The employer did not have each employee who performed work on a scaffold be trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used, and to understand the procedures to control or minimize those hazards: a) Jobsite located at 2024 North 2nd Street Philadelphia, PA 19122: The employer did not provide training to employees working on a scaffold. Employees were engaged in masonry activities while working on a scaffold, and were not trained to recognize, evaluate, and mitigate hazards associated with scaffolds including, but not limited to falls. These conditions were observed on or about 15 SEP 2020.

29 CFR 1926.451(g)(1)(vii): For all scaffolds not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, each employee was not protected by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section. a) Jobsite at 2404 North 2nd Street Philadelphia, PA 19122: The employer did not ensure that employees were protected from falls of approximately 30 feet from ground level. Employees were engaged in masonry activities and working on the end of the 6th and 7th levels of the scaffold at the southwest corner of the site without the use of a fall protection system; thus exposing employees to fall hazards. b) Jobsite at 2404 North 2nd Street Philadelphia, PA 19122: The employer did not ensure that the employees were protected from falls of approximately 24 feet from ground level. Employees were engaged in masonry activities and working on the end of the 4th and 5th levels of the scaffold at the south end of the site without the use of a fall protection system; thus exposing employees to fall hazards. These conditions were observed on or about 15 SEP 2020. Reiss Masonry Construction, LLC., was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1147076, citation number 1, item number 1a, and was affirmed as a final order on 14 JUL 2017 with respect to a workplace located at 450 Fairmount Ave Philadelphia, PA 19123.
