Flexible cords shall be connected to devices and fittings so that strain relief is provided which will prevent pull from being directly transmitted to joints or terminal screws. Employees were exposed to fire and electrical shock hazards. Employees engaged in framing activities were exposed to fire and electrical shock hazards in that the outer insulation of the flexible extension cord was pulled away from the plug end.

Employees engaged in residential framing activities using a Werner 6 foot A-frame step ladder, were exposed to fall hazards of approximately 6 feet in that the ladder was not used for the purpose that it was designed. The ladder was used in the folded position.
