29 CFR 1910.23(d)(2):  The employer did not install fixed ladders with a minimum perpendicular distance of 7 inches (18 cm) from the centerline of the steps or rungs, or grab bars, or both, to the nearest permanent object in back of the ladder:    (a)  On or about March 9, 2020, and at times prior, employees were exposed to a fall hazard while accessing the second level storage area in the northeast corner of the shop by way of the fixed ladder which was mounted 2.5 inches from the wall preventing feet from safely bearing on the ladder rungs. The rails at the top of the ladder extended only inches above the landing platform. 29 CFR 1910.28(b)(1)(i):  The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2m) or more above a lower level was protected from falling by guardrail systems, safety net system, or personal fall arrest system:  (a)  On or about February 27, 2020, and at times prior, employees were exposed to fall hazards of greater than 4 feet while access, stocking, and retrieving tires in the tire racks in the shop and in the lobby.  Fall hazards in the shop ranged from 6.8 feet to 13.3 feet.  Fall hazards in the lobby ranged from 5.8 feet to 9.6 feet.  A worker fell 7 feet from a tire rack and was seriously injured.
