29 CFR 1915.74(a)(9): The employer permitted employees to board or leave a vessel when the upper end of the means of access rested on or was flush with the top of bulwark and substantial steps was not properly secured and equipped with at least one substantial handrail approximately 33 inches in height was not provided between the top of the bulwark and the deck. On the access area of the Cape Archway, West slip location, the employer failed to ensure the steps were properly secured to prevent displacement of the steps.
