29 CFR 1926.451(b)(2)(ii): Where scaffolds must be used in areas that the employer can demonstrate are so narrow that platforms and walkways cannot be at least 18 inches (46 cm) wide, employees on those platforms and walkways were not protected from falls:  a) Work site was located at 20 Willow Street, Bayonne, NJ 07002  An employee was exposed to fall hazards of approximately sixteen feet to dirt and debris while doing roofing work from a pump jack scaffold platform approximately fourteen inches wide with no guardrails or fall protection in place.  Violation was observed on or about March 10, 2020

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used:  a) Work site was located at 20 Willow Street, Bayonne, NJ 07002  An employee was exposed to fall hazards of approximately sixteen feet to the ground. The employee was climbing on a jack bench on the scaffold pole to reach a window of the house he was working on in order to egress the scaffold platform he was working from.  Violation was observed on or about March 10, 2020

The employee was working on a steep roof with a 9:12 pitch while replacing the existing roof with no fall protection on.  Violation was observed on or about March 10, 2020
