Westwood Ave. Toledo, OH: the employer did not ensure that the employees operating the seven forklifts in the establishment were competent through the successful completion of training and evaluation as specified in paragraph (l); therefore, exposing employees to struck-by, struck against, and caught between hazards resulting from unsafe operation.

29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): A) Standard Wireway & Supply, Inc. on or about March 12, 2020, at the address of 362 N. Westwood Ave. Toledo, OH: The employer did not ensure the Accurpress hydraulic press brake (Model #717512, Serial #7907) was adequately guarded. Employees operating the hydraulic press brake were exposed to point of operation injuries and caught-in hazards during bending processes of the sheet metal in fabrication operations.
