29 CFR 1910.37(b)(4): Signs were not posted along the exit access indicating the direction of travel to the nearest exit and exit discharge when the direction of travel to the exit or exit discharge was not immediately apparent:    In the warehouse production area, signs were not posted in accordance with the standard to direct employees to the nearest exit/exit discharge where the direction of travel was not immediately apparent, exposing employees to a fire hazard.

29 CFR 1910.178(q)(1): Any power-operated industrial truck not in safe operating condition was not taken out of service, and/or repairs were not made by authorized personnel:    Employees were exposed to electrical shock hazards.  Employees engaged in material handing activities using the Yale Rider Pallet Jack, serial number B896N06789K, were exposed to electrical hazards in that forklift was not taken out of service when it was found to be unsafe and in need of repair.  The electric charging cord outside protect layer was torn and exposed the internal wires.

29 CFR 1910.303(b)(1): Electrical equipment was not free from recognized hazards that were likely to cause death or serious physical harm to employees, in that the employer did not ensure the safety of the equipment pursuant to the considerations in sections (i) through (viii) of 29 CR 1910.303(b)(1):    Employees were exposed to electric shock. Employees engaged in charging the electrical pallet jacks were exposed to electric shock in that the following instances:      a) The electrical outlet used for charging the pallet jacks had evidence of electrical arcing, as indicated by the burn marks.     b) The electrical pallet jack had indications of electrical arching at the attachment plug for five out of the six pallet jacks.
