29 CFR 1910.95(c)(1): The employer did not administer a continuing, effective hearing conservation program as described in 29 CFR 1910.95(c) through (o) whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level of 85 decibels measured on the A scale, or equivalently a dose of fifty percent.  a) 7250 Paschall Avenue Philadlphia, PA 19142, Wire Granulating Room; The employer did not administer a continuing, effective hearing conservation program for employees exposed to noise levels above 85 dBA. An employee working in the Wire granulating Room was exposed to continuous noise level of 92 dBA for an 8-hour TWA, which is equivalent to a dose of 132.80%.  This exposure exceeds the Action Level of 85 dBA.  Sampling time was 390 minutes.  Zero exposures was assumed for all unsampled time periods. 29 CFR 1910.95(i)(4): The employer did not provide training in the use and care of all hearing protectors provided to employees:  a) 7250 Paschall Avenue Philadelphia, PA 19142, Wire Granulating Room; The employer did not provide training in the use and care of all hearing protectors provided to employees. An employee working in the Wire granulating Room was exposed to continuous noise level of 92 dBA for an 8-hour TWA, which is equivalent to a dose of 132.80%.
