29 CFR 1926.152(e)(4): The dispensing units shall be protected against collision damage. a) Stonegate Construction Inc., at a jobsite located on Canal Street, Nelsonville, OH 45764: On and preceding March 12, 2020, the employer did not ensure that fuel tanks were protected from collision damage. Employees refueled equipment from a 2000 gallon tank where the tank was not protected from damage.

29 CFR 1926.652(a)(1): Each employee in an excavation shall be protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section except when: a) Stonegate Construction Inc., at a jobsite located on Canal Street, Nelsonville, OH 45764: On and preceding March 12, 2020, the employer did not ensure that employees who worked in trenches were protected from cave-ins. An employee was installing reamers and drill heads to a directional boring machine in a trench 5 feet 4 inches deep.
